An endovenous laser treatment or EVLT (to some also known as ELAS or endovenous laser ablation of the saphenous) is a treatment available here at Advanced Aesthetics, with use of the Dornier laser, that helps to eliminate bulging saphenous and varicose veins. Varicose veins result from a venous insufficiency that causes reflux or backflow of blood within the veins. As the blood flow reverses direction a swelling results within the vein which causes it to gravitate to the skins surface and bulge. EVLT is a laser treatment can be done in an in office setting under local anesthesia. Once our physicians have decided that EVLT is the best vein removal procedure for you, the varicosity will then be mapped out under ultrasound. The vein(s) being treated are then marked and the leg is draped for the procedure; local anesthetic is then injected to numb the area to be treated.
The treatment begins with a needle puncture that is made into the vein using ultrasound. A laser fiber is then inserted into the puncture, and advanced through the length of the damaged vein. EVLT works by means of thermal destruction to the venous tissue, therefore the laser energy is delivered through the laser fiber. As the laser is fired, it works to deposit high amounts of heat energy into the blood and venous tissue. This heat results in irreversible localized tissue damage, damage that shuts the vein down.
Heat is delivered continuously via the laser as the fiber is gradually withdrawn along the track of the vein until the entire vessel is treated. Once the treatment is complete, the fiber is withdrawn and the leg is wrapped with an elastic bandage after which a compression stocking is applied. The elastic bandage will remain in place for 3 days and the compression stocking should be worn by patients for at least one week. If additional smaller veins remain following the EVLT treatment, sclerotherapy can be done a month or so after the procedure to give patients an aesthetically pleasing result.
